Zakat / Sadaqah Assistance

1. Please complete this form fully and provide all information requested.
2. Please provide copy of a valid and current government issued photo ID.
3. Please attach the copy of your current tax papers, Credit card statement, Bank Statements and the tax 4. You may use the reverse side of this form if additional space is needed.
5. Submitting the application does not guarantee approval of the assistance in part or in whole.
6. All information on this form will be treated confidentially.
7. Please allow up to 3 weeks to assess and process your application.
8. Upon completion of your application assessment, SNMC Zakaat/Sadaqah Committee will communicate with you in writing (preferably via email or mail).
9. We are not able to assist with sending money abroad or with cases outside of the National Capital Region.
10. Cash will not be provided.
11. Bills and Rent payments will be made directly to the Landlord or Utility Company. Please include this name in your application.
12. Follow up is mandatory and will be conducted after 3 months once the case is resolved for all the approved cases by the committee.
